Tax Forms at the Library

Each year the library receives tax forms for the public from the Internal Revenue Service (IRS) and the Massachusetts Department of Revenue. The Massachusetts Department of Revenue sends the library a limited number of state tax forms. The library has no control over the number of forms received. Forms are available on a "first come, first served" basis and cannot be held aside.

Tax forms are located on the second floor of the library, in the back alcove near the microfilm machines. Because of the large volume of forms and the great demand for them on a daily basis, library staff will not be able to tell you which forms are there at any particular moment. However, most forms, both federal and state, can also be found online (see below for instructions on downloading online forms). You can also obtain forms by calling 1-800-829-1040 (federal individual), 1-800-829-4933 (federal business) or 1-800-392-6089 (state). Reproducible federal tax forms are also available in a binder in the Reference Department.
